  • Click here to fill out and return (mail or fax) the registration form.

  • A $25.00 deposit will hold your spot in the class. The entire amount due must be paid in full to the Synergy Office no later that 7 days prior to the class.  All classes have limited enrollment. All registrations are on a first-come, first-serve basis. Please arrive early to sign-in on the first day of class
  • SHACMS does not assume responsibility for any loss or damage to persons or property in connection with any CEU class.
  • SHACMS reserves the right to cancel any class due to insufficient enrollment or emergency.
  • In the event that your class is canceled by us you will receive a refund of the full amount that was paid. SHACMS is not responsible for any financial loss due to cancellation, including, but not limited to travel expenses and accommodations.
  • In order to receive a refund of your tuition (minus non-refundable deposit) SHACMS must be notified prior to the start of the class that you will not be in attendance.
